I WOULD LIKE/WOULD NOT LIKE for TRAH to use photo(s) of my pet(s) on Facebook, Instagram, and/or TRAH website for advertisement/educational/promotional purposes.

The undersigned agrees, whether he/she signs as agent or owner, that in consideration of the services to be rendered to the patient, he/she hereby individually obligates himself/herself to pay the account of the hospital in full at the time services are rendered. Should the account be referred to an attorney or collection agency for collection, the undersigned agrees to pay all attorney's fees and collection expenses. All delinquent accounts shall accrue interest at the rate of 1.5% and a $6.00 billing charge per month.
